Femto-Lasik

Femto-LASIK is an effective and popular refractive procedure for vision correction. It can greatly reduce dependence on glasses or contact lenses, enabling patients to live a more active lifestyle.

Combining both Femto corneal flap  and excimer laser vision correction techniques, LASIK (Laser In Situ Keratomileusis) is a dramatically effective technique for correcting conditions including myopia (nearsightedness), hyperopia (farsightedness), and astigmatism. We always strive to provide the most advanced technologies to our patients and proudly offer the Alcon's WaveLight® EX 500 Excimer Laser System in Faisalabad.

Patients over the age of 18 who have not had a significant change in vision and can see clearly with contact lenses or glasses is a good candidate for laser vision correction. Recent innovations in technology are allowing an increased number of people to qualify for the surgery, however people with certain medical histories or conditions are not be good candidates. We will help you learn about the various treatment options and also decide whichever option suits you most.


 

The new Femto-LASIK is a computer-controlled Blade-FREE LASIK technology which works by delivering rapid pulses of light, a quadrillionth of a second each, to a pre-programmed depth and position within the cornea. Each pulse forms a microscopic bubble. As the Femto laser moves back and forth across the eye, the bubbles are then connected to form the corneal flap. Hence in Femto-Lasik there are two lasers used to perform the treatment.

Also in contrast to a microkeratome, which simply delivers a one-dimensional cut, the blade free Femto-LASIK procedure enables the eye surgeon to precisely design and control many aspects of the flap itself, such as its thickness, its circumference, and the angle of its edges, effectively customizing the flap to the individual patient's eye. This is of particular benefit to patients who have steep, flat, or thin corneas, as they may not have been candidates for traditional LASIK surgery.

Lasik

Traditional LASIK surgery uses an instrument called a microkeratome for the creation of corneal flaps. The microkeratome is a fine, oscillating, hand-held steel blade that creates a cut as it travels across the cornea. While effective, the microkeratome has not changed significantly in 30 years of use in refractive surgery, and the quality of the outcome relies heavily on the skill of the individual LASIK surgeon performg the procedure and can be sometimes compromised.

Before the invention of Femto laser traditional LASIK was used to treat refractive errors but now with the advent of newer technologies this traditional LASIK is gradually becoming obsolete because of higher risks and safety issues.

 

THE DAY OF TREATMENT:

You are first welcome by the Laser Councellor, who will perform Corneal scans related to LASIK. The you will receive a complete eye examination and consultation with Javed Eye Centre doctors. During this time, you will have an opportunity to meet with Dr. Muhammad Javed or Dr. Badar Javed and inquire about any of your concerns. These doctors will discuss your particular situation and help determine the best course of action.

On the procedure day, our staff will administer some anesthetic eye drops to numb the eye. When you are ready, we will perform the LASIK procedure.

First of all a flap is created by a Femto Laser and with the use the Alcon WaveLight® EX 500 Excimer laser microscopic amounts of corneal tissue are removed that correspond with the map of your cornea. In a few seconds, the correction will be complete, and the corneal flap will be replaced into its original position.

LASIK is a common form of laser vision correction because the healing process is greatly expedited by the corneal flap. It allows the treated portion of the eye to remain covered, providing much faster healing than other forms of laser vision correction.

 

 

Progress Pattern:

Most patients generally recover very quickly following Femto-LASIK treatment (within a few hours).

Many people report that they can see clearly the next day, and that they experience very little discomfort after LASIK. This is because with LASIK the corneal flap is placed over the area of treatment, which resists causes of irritation during the healing process.

Side effects of LASIK are mild and temporary. Some of these may include dry eyes and an increase in halos and glare; it should be noted that people with large pupils or more severe refractive corrections are more likely to experience these effects. As the eye healing occurs, these symptoms generally disappear.

Photorefractive Keratectomy PRK

PRK or photoreactive keratectomy, is a trusted laser vision correction procedure used by us for those patients who are cant have LASIK for some reasons.

 

 

Procedure for PRK laser vision correction is very similar to that of LASIK. The main difference is that instead of creating a flap in the cornea a thin layer of epithelium is removed. This layer regenerates quickly within a few days.

Once the inner layer of the cornea is exposed, the laser vision correction proceeds. Excimer Laser reshapes the cornea according to the patient’s specific prescription. Afterwards a bandage contact lens is placed on the eye, to protect it for three or four days after the surgery and to allow the surface epithelium to heal undisturbed.

After undergoing PRK laser vision correction most of the patients report feelings of sensitivity to light, dry eyes, and halos around light sources. These symptoms are temporary, and go away on their own within four to six days. Immediately following PRK, patients may notice that their eyesight is blurred.  It can take up to a few weeks to reach full visual recovery, depending on the length of time needed for the surface cells to regrow.

The level of discomfort after PRK is usually higher than in LASIK, because in PRK there is no corneal flap to cover and protect the tissue. The level of pain is moderate depending upon a patient’s threshold and can easily be controlled with pain killers. For sensitive patients, stronger pain medication can be advised.

It takes longer to achieve recults with PRK than LASIK because it takes longer for the epithelium to regenerate and restored. LASIK can often produce clear vision within hours while PRK may take several days or even weeks to gain clear vision.

 

Inspite of this difference, studies have been performed to demonstrate that there is absolutely no difference in the final visual results when comparing PRK and LASIK. It is hence a wonderful alternative for patients who can not have LASIK, or for those with certain occupational requirements or with specific corneal conditions.

The WaveLight® EX500 is the fastest excimer laser available today, operating at 500 Hz with a treatment time of 1.4 seconds per diopter-a 20 percent improvement over previous laser generations. The faster laser head reduces the incidence of corneal stromal bed dehydration during the procedure, which can contribute to overcorrection and occurs when a flap is created in the eye.

The faster speed, combined with a precise algorithm, results in less stromal drying of the cornea during treatment, increasing patient comfort and decreasing patient fixation fatigue. This video-based eye tracker ensures that sensitivity to patient eye movement is minimized, featuring a 2 millisecond latency time and the ability to track a wide range of pupil sizes from 1.5mm to 8mm. The tracker also self-regulates, adjusting the laser's speed as needed. The laser calibrates itself automatically.

Alcon's patented PerfectPulse Technology ensures that the WaveLight® EX500 can do what other existing lasers in the market cannot. It delivers an extremely precise pattern of pulses to the periphery of the cornea in order to minimize spherical abberations, reducing the risk of nighttime glare or halos in patient vision. Maintaining a high pulse frequency while minimizing thermal tissue damage provides improved precision and safety to ensure the best treatment results for patients.

The WaveLight® EX500 also has the ability to measure and monitor corneal thickness (pachymetry) before, during, and after the procedure. The availability of online optical pachymetry ensures the surgeon is always aware of a patient's corneal thickness throughout the entire procedure.

Custom LASIK provides a superior quality of vision by tailoring treatments to very unique aspects of an individual's eye. We use the Wavelight EX500 Laser to reshape the cornea. The laser allows for wavefront optimized treatments which treat not only nearsightedness, farsightedness, and astigmatism, but those distortions that are unique to a patient's eye. These distortions are called higher order aberrations.

Custom LASIK reduces visual symptoms such as glare and halos when compared to non-custom treatment, or 'conventional' treatment. For example, patients who have custom LASIK report less glare with oncoming headlights when driving at night, and less halos around streetlights. The quality of vision after custom LASIK is described by researchers as superior “contrast sensitivity”. The satisfaction rate in patients who have Customised LASIK is excellent.

Post Refractive Laser Treatment Instructions
  • DO NOT RUB THE OPERATED EYE!
  • If you should experience persistent or severe pain in the operated eye or progressive loss of sight in the eye, sudden flashing lights or burst of floaters, contact us immediately. Call 0332-9660888 during business hours, or at night. The manager will arrange to see you immediately, and can get in touch with the doctors.
  • Wear goggles to bed at night for two weeks and continue wearing glasses during the day.
  • You may sleep on either side with as many or as few pillows as you wish as long as you are wearing the goggles.
  • Wash hair with head back only for one week. It's OK to shower, but avoid soap or shampoo in the operated eye, and be careful not to rub it in the shower.
  • Avoid lifting above 10 pounds for two weeks (if you are straining very hard to lift, it's too heavy; if it's easy to lift, it's OK.)
  • Allow at least 5 minutes between using the different drops. You can gently pull down your lower lid, pressing on the cheekbone, to get more room to put in the drops, but don't touch your upper lid.

Unless otherwise directed, For refractive surgery Patients:

  • Follow-up four days after surgery and next week
  • Pred-Forte (steroid; milky liquid; white cap) & Vigamox (antibiotic; yellowish liquid) - one drop of each Four times daily during waking hours till your first checkup.
  • Systane Eye Drops (Large bottle), Four times daily.
  • Tablets to be used as directed.
